Case Management Nurse

Pay Range: $2250 - $2750 Gross Weekly

Job Description:

• We are seeking a dedicated and experienced Registered Nurse (RN) to join our team as a Case Manager. The ideal candidate will have strong experience in case/resource management, utilization management, and care management. This role requires someone with a passion for improving patient outcomes and providing high-quality care.

Key Responsibilities:

• Manage a designated caseload of patients, coordinating care across multiple providers and settings to ensure optimal outcomes.
• Conduct assessments and evaluations of patient care needs and develop comprehensive care plans.
• Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure effective care delivery.
• Monitor and track the progress of patient care plans, adjusting as necessary to meet changing needs.
• Advocate for patients and their families, ensuring they have the resources and support needed for recovery.
• Ensure appropriate utilization of healthcare services to improve patient care while minimizing costs.
• Maintain accurate and up-to-date documentation in accordance with hospital policies and regulatory requirements.

Required:

• Active New York State RN License with Primary Source Verification.
• AHA BLS (Basic Life Support) certification.

Experience:

• Minimum of 2 years’ experience in case/resource management and utilization management is required.
• 3-5 years of experience in care management is preferred.
• EPIC system experience required.
• PRI (Patient Review Instrument) and SCREEN experience/certification is ideal.
