Job Details

Coordinate cost-efficient, reimbursable patient care under the insurance plan and physician approval.
Review insurance information, verify eligibility, and ensure services align with the care plan.
Oversee and ensure compliance in clinical documentation, care coordination, and billing processes.
Participate in QAPI activities and chart audits to uphold high-quality care standards.
Supervise and support pediatric nurses through joint visits, evaluations, and development programs.
Communicate effectively with the RN Case Manager and interdisciplinary team to optimize patient care.
Facilitate patient discharge planning and ensure thorough documentation of all care coordination activities.

Skills You Need

Licensed and registered as a Registered Professional Nurse in New York State..
BLS certification required.
3+ years of experience in a home care/CHHA environment, with supervisory experience preferred.
Prior experience with pediatric care and full vaccination for COVID-19 required.
Reliable transportation within Brooklyn, Queens, and Manhattan, and the ability to work holidays and weekends on a rotating schedule.
